Discussion . This study shows that, in ~50% of immunocompetent children with nontuberculous mycobacterial cervical lymphadenitis, the indurative reaction to M. tuberculosis PPD measures ⩽15 mm by the ball-pen method, and in ~25%, it measures <10 mm. Although large indurations have been found among adults infected with various nontuberculous mycobacterial species [ 4 ], the incidence of large indurations found here is greater than that reported elsewhere in series of children with cervical lymphadenitis. Wolinsky [ 5 ] reviewed results of 105 American children with presumed nontuberculous mycobacterial lymphadenitis (83 culture-proven cases), of whom 91 underwent a PPD skin test; only 7 children (8%) showed indurations of >15 mm, whereas 57 (63%) had indurations of ⩾10 mm. In another study from Greece, among 43 of 47 pediatric inpatients with presumed nontuberculous mycobacterial lymphadenitis (40 culture-proven cases) who underwent a tuberculin test, the median induration was 7 mm (range, 0–15 mm), and most patients (70%) had indurations of <10 mm [ 6 ].

Discussion

Differences between our results and those from different parts of the world [ 5 , 6 ] may indicate the existence of different nontuberculous mycobacterial species and subspecies. This diversity may result from regional environmental factors that affect mycobacterial cell wall structure and, consequently, immunogenicity.

In fact, in contrast to previous reports among children with nontuberculous mycobacterial lymphadenitis [ 5–7 ], we had a high isolation rate of M. haemophilum. This might be the result of a high prevalence of this species in our geographic area, but it may also be attributable to our laboratory routine of use of broth with an iron supplement incubated at 30°C for processing aspirated specimens [ 3 ]. Correa and Starke [ 8 ] claimed that the size of reactions to M. tuberculosis PPD in infections caused by M. avium-intracellulare complex is usually 0–10 mm and rarely exceeds 18 mm, whereas infections caused by other species, especially Mycobacterium marinum and Mycobacterium fortuitum , are frequently associated with reactions of 10–20 mm. Because of scant data, it is impossible to estimate the range of indurative responses to M. tuberculosis PPD in children infected with M. haemophilum in other parts of the world. Nevertheless, our series shows that this species can occasionally be very immunogenic.

Elliptic curve cryptography (ECC) is one of the most powerful but least understood types of cryptography in wide use today. An increasing number of websites make extensive use of ECC to secure everything from customers' HTTPS connections to how they pass data between data centers. Fundamentally, it's important for end users to understand the technology behind any security system in order to trust it. To that end, we looked around to find a good, relatively easy-to-understand primer on ECC in order to share with our users. Finding none, we decided to write one ourselves. That is what follows.

Be warned: this is a complicated subject, and it's not possible to boil it down to a pithy blog post. In other words, settle in for a bit of an epic because there's a lot to cover. If you just want the gist, here's the TL;DR version: ECC is the next generation of public key cryptography, and based on currently understood mathematics, it provides a significantly more secure foundation than first-generation public key cryptography systems like RSA. If you're worried about ensuring the highest level of security while maintaining performance, ECC makes sense to adopt. If you're interested in the details, read on.

The history of cryptography can be split into two eras: the classical era and the modern era. These new algorithms were revolutionary because they represented the first viable cryptographic schemes where security was based on the theory of numbers; it was the first to enable secure communication between two parties without a shared secret. Cryptography went from being about securely transporting secret codebooks around the world to being able to have provably secure communication between any two parties without worrying about someone listening in on the key exchange.

Demographic and behavioral characteristics of women infected with M. genitalium. The 50 women (7.0%) who were positive for M. genitalium were significantly younger, less likely to have more than a high school education or an income, and somewhat less likely to have been married at any point (P=.06) than were the 669 women without M. genitalium infection ( table 1 ). These 2 groups were similar with respect to race. Women positive for M. genitalium were less likely to use any contraception (P=.02) and somewhat more likely to have been pregnant during the year before the sample was obtained (P=.08) or to have had a spontaneous miscarriage at any point (P=.13)

Women infected with M. genitalium were significantly younger at sexual debut, had engaged in sexual intercourse more recently, more often reported having ⩾2 new sex partners in the past 30 days or having douched at least 2 times each month, and were more likely to smoke, but less often reported ever receiving cunnilingus. Associations between M. genitalium and demographic, gynecologic, and sexual behavior characteristics were similar in magnitude and statistical significance after women with N. gonorrhoeae and C. trachomatis infections were excluded, with the exception of time since last sexual intercourse, which was no longer statistically significant (data not shown)

Clinical and microbiologic findings associated with detection of M. genitalium. Women with M. genitalium infection had significantly higher vaginal and cervical PMNL counts, more often had cervical mucopus, and were somewhat more likely to have easily induced cervical bleeding than were women without such infection (P=.06) ( table 2 ). MPC was present in 48.0% of women with and 28.6% of women without M. genitalium (P=.004). Women with M. genitalium complained significantly more often of abdominal pain, vaginal discharge described as abnormal or as brown or bloody, or nausea, diarrhea, and vomiting in the previous 30 days and were more often in the proliferative phase of the menstrual cycle (days 6–14). There was no association with uterine, adnexal, abdominal, or cervical motion tenderness (data not shown)
